Hello, this is Dr. Park Ji-seon of ACRO Korean Medicine Clinic 😊

When the face looks wide and angular, many people wonder, “Should I get square-jaw Botox?”

But not every cause of a wide-looking lower face from the front is the masseter muscle.

In some cases, fat and soft tissue around the jawline make the lower face look wider,

and in others, bone, muscle, and fat all contribute together.

So for a face that looks like a square jaw, it is important not only to look at face shape alone

but first to look at which tissue is creating the current contour.

Today I will show a before-and-after case of one ONDA (microwave lifting) session

for a woman in her 30s whose concern was a wide, angular lower face.

What Brought Her In?

This patient’s main concern was a lower face that looked wide and angular from the front.

In the before photo, the width of the lower face running from below the ear to the chin tip is relatively wide,

and the jawline comes down in a straight line, so the lower part of the face looks angular.

But there was one more thing I considered important in this patient’s face.

It was the hollow beneath the cheekbones.

The area below the cheekbones already has relatively little volume,

while farther down, the volume of the lower face increases again.

In a face like this, if we simply took the approach of “let’s reduce the face fat a lot,”

the volume of the area below the cheekbones, which is already sunken, would be reduced even further,

which could actually emphasize the hollow and make the contours of the face stand out more.

So in this treatment, an important goal was to refine the lower face and jawline while preserving the hollow beneath the cheekbones.

For Facial ONDA, “Where to Preserve” Matters as Much as “Where to Reduce”

In ONDA Lifting consultations, many people ask, “Is the effect better if more energy is delivered?”

But on the face, it is hard to evaluate a treatment by total energy alone.

This is because even with the same amount of energy,

the effect on the facial contour can differ depending on which areas it is distributed to and how.

In particular, the distribution of fat differs from one area of the face to another,

so there are places where reducing volume helps, and places that are already sunken and actually need to be preserved.

How Was This ONDA Lifting Done?

This treatment was carried out as follows.

ItemDetails
Treatment dateSeptember 10, 2026
SessionSession 1
Output80W
Total energy65,000J
7mm handpieceJawline, deep cheeks (excluding the hollow cheek area)
3mm handpieceMainly cheeks and jawline
Treatment designThe treatment range was adjusted so that excessive fat reduction would not occur in the hollow area beneath the cheekbones

How Did the Lower Face Change Right After One Session of ONDA Lifting?

Right after one session of Facial ONDA

The photos above were taken on the same day, before ONDA Lifting and right after one session.

When looking at the photos, the first thing to compare is not the overall size of the face but the silhouette of the lower face running from below the ear to the chin tip.

Before treatment, both sides of the lower face extended relatively wide outward, so the lower part of the face looked angular from the front.

Right after treatment, the contour around the jawline was tidied, and we can see the previously wide-looking lower face converging a little more softly toward the chin tip.

The point of these photos is that, as the volume and contour on the outer side of the lower face were refined, the overall impression of the lower face looks different.

How Did the Jawline Change When Seen from the Side?

Right after one session of Facial ONDA

In the side photos, compare the line running from the chin tip to the neck.

Before treatment, the volume under the chin continued gently down to the neck, so the boundary of the jawline looked blurred.

Right after treatment, the area under the chin was tidied, and the angle between the chin and neck appears more distinct than before.

For a Square Jaw, Which Is Right: Botox or ONDA?

Not every treatment suits everyone who has a square-jaw look.

If the development of the masseter is strongly affecting the width of the lower face, treatment that targets the muscle can be considered.

Conversely, if fat and soft-tissue volume of the jawline and lower face are what make the face look wider,

then how to refine that volume may matter more.

Of course, in real faces, bone, muscle, and fat often all contribute together.

So rather than deciding simply, as in “it’s a square jaw, so Botox” or “there’s face fat, so ONDA,”

a process of first distinguishing what is making your own lower face look wide is needed.

Clinical Points to Note in This Case

First, the cause of a lower face that looks like a square jaw must be sorted out first.

When the masseter is developed and makes the face look angular, treatments such as Botox that target the muscle are suitable,

but when the fat layer along the jawline and under the chin is thick, a treatment that works on fat, such as ONDA, may be suitable.

In a lower face with a large share of fat layer, like this patient’s, the change in line can be confirmed in photos even right after a single session.

Second, even within the same cheek, the places to take away and the places to protect are different.

For the jawline and deep cheeks, volume was reduced with the 7mm handpiece, while the hollow cheek area was excluded from the 7mm treatment area and only skin elasticity was reinforced with the 3mm handpiece.

In Facial ONDA, we believe that what determines satisfaction is closer to this range design than to the total amount of energy.

Frequently Asked Questions

Q. Will my square-jaw line change with just one ONDA session?

If the square jaw is not caused by bone and muscle, a change in the lower-face line can be seen even right after one session, as in this case.

However, the photos right after treatment also include temporary swelling, so at our clinic we recommend 3 sessions at 3-4 week intervals and judge the result by how things look after the sessions are complete.

Q. My cheeks are somewhat hollow. Can I still get Facial ONDA?

Yes, you can. The hollow cheek area is left out of the deeper-layer treatment that reduces fat, and the range is adjusted so that only elasticity is reinforced in the shallow layer.

Q. Are redness and a feeling of heat after treatment okay?

Redness and a feeling of heat can appear right after treatment, so at our clinic soothing care is given immediately afterward. Burns or lumps (nodules) may rarely occur, so if you have pain or a lump you can feel after treatment, please contact us right away.

In Closing

When a squared lower face has fat along the jawline and under the chin mixed in,

just refining that fat layer with ONDA can make the face line rounder.

This patient was treated in one session at 80W and 65,000J, and a change in line was confirmed right after treatment in both the front and side views.

If you are considering square-jaw Botox but can feel soft flesh when you pinch the area,

or if you are bothered by both a double chin and a square jaw, it is better to first check whether a fat layer is the cause.

At ACRO Korean Medicine Clinic, we look at the thickness of the fat layer along the jawline and under the chin, areas that need to be preserved such as hollow cheeks,

and skin elasticity together, and set the treatment range for each handpiece.
